This position sits within the Covid Response structure, Clinical Pathways Team and is a key role in the provision of care needs assessment and care plans to community members who contract coronavirus.
The Care Pathway Navigator provides telephonic social and welfare needs supports to covid positive clients during their illness.
The Care Pathways Navigator coordinates access to services that support their covid recovery care plan.
The Care Navigator will be able to engage clients to deliver tailored supports based on person centred plans, using a strengths-based approach with trauma informed principles as required. They will also identify gaps in clients support provisions and use a person-centred approach to work collaboratively with the client to implement their care plan.
They will have the ability to build rapport and develop high quality professional relationships with clients, formal and informal supports, involving them in the implementation of their care plan.
They will conduct medical screens based on symptoms connected to coronavirus, referring the client into appropriate care pathways, and liaise with GPs of clients to enable them to monitor the wellbeing of their clients who are COVID positive.
Establishing and maintaining sustainable service pathways, partnerships and support networks to meet client needs by engaging informal supports, formal network key stakeholders and emergency service professionals as required will be an important part of the role. Additionally they will provide time limited support coordination to assist the client to connect with supports, whilst they recover from coronavirus.
The Care Navigator must liaise with partners such as RMH intake, Family Violence providers, AOD providers, local council and other specialist providers to establish social care plan that enables quarantine, as well as liaising with Department of Health and Human Services as required regarding the status and care of clients who are COVID positive.
This position requires a Victoria Police Check.
Care pathways coordinators will come from the following disciplines: Nursing, AOD, Mental Health, Family Violence, Community Development, and Other relevant Allied Health Disciplines
Care pathways Navigators operate 7 days per week with shift hours varying depending on service need, incumbents will be required to participate in shifts as rostered. You will be required to undergo regular COVID testing and provide cohealth with the results.
