To support and further enhance the care and services agenda, we are now looking to appoint a high-calibre Manager, Clinical Care & Services (Registered Nurse) as the organisation embarks on the next phase of growth.

About The role

Based at the Port Lincoln Head Office and reporting to the Executive Manager Home Care Programs, while also working in close collaboration with the broader West Coast Homecare team, the role is to provide highly efficient, accurate and proactive daily management of consumer outcomes across the region in the delivery of clinical care, personal care, and other support services within the West Coast Homecare scope of practice. You will manage the end-to-end client journey, with a focus on ensuring the wellbeing of client needs, develop care plans, work collaboratively with care staff and monitor care provided with a strong focus on consumer dignity and respect, valuing their identity, culture and diversity, in-turn leading to high-level client satisfaction, while maximising outcomes for clients.

Your specific responsibilities will include;

  • Leadership, management and coordination of activities to Home Care Coordinators, Community Care Workers and Service Providers
  • Development, implementation and evaluation of patterns of patient care for in-home aged care and disability support services
  • Accountable for clinical care and services, operational planning and decision making for in-home aged care and disability support services
  • Implementation of appropriate clinical standards, through quality assurance programs in-home aged care and disability support services
  • Contribute to the development of nursing and service delivery policy to enable the provision of quality nursing care and services
  • Compliance with the delivery of safe and quality care and services in accordance with regulatory and program requirements, Age Care Quality Standards, the NDIS Practice Standards, Charter of Aged Care Rights, the NDIS Practice Standards and Code of Conduct and the Nurses Act

About You

You will be a Registered Nurse with extensive experience caring for people (Home Care experience preferred) with in-depth knowledge of the Aged Care & Disability Legislation. You will have demonstrated management in a human service organisation with leadership of a home care team preferred. You will have a warm and friendly approach to building lasting relationships whilst demonstrating exceptional organisational and communication skills. You will enjoy supporting and developing care staff to deliver exceptional care. You will be a self-starter with attention to detail, well-developed planning and organisation skills, strong engagement, written and verbal communication and high-level of computer literacy. Supporting all of this are your highly developed emotional & social intelligence competencies.
