Due to strong demand for our Home Care Packages we need an experienced Case Manager to join our team.

About the role:

You will be responsible for managing a caseload of clients and will need to assess, plan, implement, monitor and evaluate care plans.

This role is supported by our Home Care Packages Manager and an established scheduling & customer care team.

Who we are looking for:

We need someone who has:

  • Experience in community-based client assessment, goal-based care plan development and implementation;
  • Case management and documentation experience and the ability to develop and implement care plans;
  • 3yrs + experience in Case Management Demonstrated understanding of relevant legislation for Home Care Packages and Aged Care Quality Standards;
  • Strong interpersonal and communication skills including excellent written and verbal communication skills;
  • Excellent community connections and knowledge of services to appropriately link clients to the help they need;
  • Proficient with client management systems, Microsoft office and relevant government funding portals;
  • A 'can-do' attitude & a passion for helping people stay living independently.
