  • Recovery and Rehabilitation Worker | Wagga Wagga
  • Continuity of Support & Well Connected Program

Wellways has a number of programs providing services within the Murrumbidgee consisting Mental Health Rehabilitation & Recovery Services, Suicide Prevention, grass roots education and training in mental health and a range of Peer Facilitated Education Programs.

The Well Connected program is designed to work with participants who experience severe mental illness or concerns, to address key areas that support them to participate in their communities. This support often results in improvements in their quality of life and, most importantly, assists in their recovery from mental health issues. A key outcome will be supporting the participant to identify and nurture natural supports that sustain recovery and assist in the development of self-management skills.

The key target populations of Well Connected are: people who identify as Aboriginal / Torres Strait Islander or are 65 years and older and who experience severe mental illness or concerns.

Using Wellways' Community Recovery Model, the Recovery and Rehabilitation Worker will provide short term support to people with a mental illness through Well Connected.

The goals of the program are to:

  • Work with people to explore multiple areas of life and community spaces that interest them, not restricted by what others believe is possible or desirable
  • Support people to take the lead in making choices and decisions about things that are important to them, including managing any risks that may be involved
  • Promote participation that happens in the same places everyone else in the community can access, and maximise opportunities for connection with others
  • Offer evidence-based support technologies that enable participation, including peer support, engaging family and friends, and natural support development
  • Support families and natural supports to sustain their role, and to pursue wellbeing and inclusion in their own right
  • Work directly with community members and groups to establish welcoming and mutually supportive and spaces for all people.

Our Practice Frameworks identify a range of tools and processes to assist participants to develop the confidence and capacity to manage their own wellbeing; equip family and friends with information and skills; and engage community members as mentors and supports.

The Wellways Practice Frameworks are:

  • Community Recovery Model
  • Model of Care
