The successful applicant will be able to:
- Cultivate and coordinate an Aged Care Model of practice, optimising individual funding with emphasis on consumer directed care and strengths based approach
- Work in partnership with relevant internal and external stakeholders to ensure organisational responsiveness to changes in the Aged Care Sector
- Manage the service development process, product design, associated research, implementation and evaluation
- Identify and refer opportunities for innovation service development through project management
- Provide expert advice, guidance and support to all relevant stakeholders to ensure services are delivered compliantly under relevant Aged Care legislation and regulatory obligations
- Manage recruitment and professional development of the Aged Care workforce through a planned and consistent approach, ensuring workforce availability and skills to meet client needs, and deliver program outputs
- Support the senior executive to manage sector funding contracts to ensure achievement of outputs and outcomes
- Ensure all record preparations, reports, submissions and correspondences in accordance with policies, procedures adhere to funding agreements, Age Care legislation, regulation and standards
Requirements:
- Minimum of three (3) years’ experience as a Manager or Coordinator within the Aged Care sector
- A detailed understanding of the Aged Care sector, particularly pertaining to CHSP, HCP and Community Visitor Scheme funding and support mechanisms
- A detailed, applied understanding of Continuous Quality Improvement relevant to Aged Care legislation, regulations and Standards
- Demonstrated understanding of Consumer-Directed Care, and the ways in which the needs of older people, people living with disabilities and other vulnerable cohorts are able to be met through individualised funding
- Demonstrated ability to apply creative thinking to institutional services
- Willingness and ability to support the promotion of VGC’s CHSP, HCP and CVS services, and therefore grow VGC’s Aged Care portfolio
- Well-developed team management and leadership skills; the ability to inspire and motivate colleagues; and the drive to act as a change agent within the organisation
- Have a professional and strengths-based approach to supporting all recipients of the organisation’s services
- Familiar with alternative care models such as culture, relationship or inter generational centred care
- Demonstrated understanding of and ability to carry out high-level case management duties
- Experience in the Microsoft suite of programmes with demonstrated proficiency in Word and Excel
