ABOUT THE ROLE:

We are currently recruiting Support Coordinators to join the Support Coordination Team.

As a Support Coordinator you will be responsible for implementing NDIS plans into action whilst linking and support participants to enable accessibility, inclusivity and empowerment into the wider community.

AS A SUPPORT COORDINATORS, YOU WILL:

  • Build strong relationships with participants, NDIS planners and service providers
  • Support NDIS participants, their families and carers to understand and engage in the NDIS
  • Coordinate NDIS supports in relation to individual plans whilst maintaining a person-centred, capacity building and goal-focussed approach
  • Support participants to build their capacity in maintaining positive relationships with their supports
  • Monitor billable hours and ensure that claims for payment are lodged accurately and in a timely fashion
  • Maintain accurate records of work carried out, in accordance with legislative and audit requirements
  • Produce progress reports in accordance with NDIA requirements
  • Be part of a dynamic and dedicated team

ABOUT YOU:

Ideally, this role would suit an experienced Support Coordinator with demonstrated experience in managing a complex caseload, dealing with crisis situations, finding and developing solutions and writing assessments and reports.

If you have a positive, resilient and flexible attitude, enjoy working in a fast-paced environment, can move easily between participant supports and administrative work we would love to hear from you!

Relevant qualifications in Social work, Community Development, Disability Studies or Human Services equivalent, as well as competency in Auslan and an understanding of access and equity issues for metropolitan and regional deaf and hard of hearing people, with and without additional disabilities, will be well regarded.
