VIA Care is looking for a support coordinator to work with a diverse range of participants, ranging from children to older adults and from a range of cultural and linguistic backgrounds.

VIA Care is dedicated to bringing a trauma informed and family inclusive practice approach to disability support and recognises that disability supports must work collaboratively with other formal and informal supports.

VIA Care is an organisation undergoing significant growth and operates across Melbourne with locations including Brunswick, Fitzroy and is due to open a location in Box Hill in late 2021.

The role

We are looking for a candidate who can undertake ongoing needs analysis with participants and coordinate disability services which meet the needs of clients, whilst working in collaboration with other involved services and in the context of the participant’s family and social network.

Working as a core part of the team you will be supporting participants to develop and achieve goals which improve their quality of life and increase their independence. You will be working with stake holders comprising people with disability, their families and networks who present with a variety of needs or service requests, purchasers and service providers.

We are seeking a candidate who has the ability to explore informal support opportunities which meet the specific interests and goals of our participants and can work proactively to develop linkages as required.

You will be required to operate across different levels of Support Coordination with each individual you support and monitor plan expenditure to maximise the impact of funding available to the participant.

Attention to detail and administrative ability is essential as you will be responsible for the compilation of accurate and comprehensive documentation, and continually gather information regarding participant progress and their changing needs for work in future planning.

Key tasks

  • To support individuals to navigate the service system and explore a range of service options available to them within their community.
  • To implement, monitor, and review the effectiveness of supports and negotiate supports as agreed/funded and/or as appropriate.
  • Provide a holistic coordination function which smoothly integrates supports into the life of a participant, in line with existing supports, family and social networks and leverages off existing strengths. Ensure that all activity is guided by the voice of the participant to meet their preferences and goals, and collaborate with other services which the participant is involved with or requires.
  • Manage your own time, set priorities, plan and organise your own quality work to achieve specific objectives
  • Empower participants to exercise choice and control to access a range of formal and informal mainstream and community-based support, by identifying opportunities for innovative service delivery
  • Continually seek opportunities to build the capacity of participants, their families and social networks and their support team to improve quality of life and level of independence.
  • Adhere to reporting obligations regarding incidents or concerns which may arise relating to service quality, participant safety and worker suitability.

Eligibility criteria

  • Post-secondary qualification and experience related to disability, allied health, social work or case management.
  • Demonstrated understanding of best practice in disability or an allied health setting.
  • Understanding of the NDIS or demonstrated ability to operate within a regulated and well-structured welfare setting.
