The Opportunity

We are looking for client focused and reliable individuals to provide support to NDIS funded participants with challenging behaviours and restrictive practices, promoting choice and control and assisting participants in achieving their personal goals. Supports can include assisting with daily living activities, providing domestic assistance, as well as personal care, transport, and other aspects of daily living as specified by their NDIS plan.

Key responsibilities include:

  • Provide a range of high quality support services that enable consumers to achieve their goals  
  • Work with consumers to develop individual support and/or goal plans, as per assessment tools as required, within agreed timeframes
  • Adherence to service agreement requirements of the funded package & meeting of deadlines.
  • Establish rapport and build supportive relationships using person-centred and strength-based approaches.
  • Observe and report on clients ‘health and wellbeing’ concerns and assist clients to review clients Service Plans goals.

The successful candidate will have a Minimum Certificate IV in Community Services, Disability, Mental Health or related field, and experience in working with individuals with challenging and complex behaviours.
