THE OPPORTUNITY

The purpose of this role is to assess and monitor individual needs by delivering quality client care, under the direction of the Clinical Nurse. This position will supervise activities of the Enrolled Nurses and Personal Care Attendants.

KEY RESPONSIBILTIES:

• Assess, monitor and provide information relating to ACFI requirements for the Interim Care Plan
• Identify the health needs of residents through clinical assessment
• Develop resident care plan based on clinical assessment 
• Provide quality residential care at all times with residents receiving clinical care, according to their assessed needs, that reflects the Standards and Guidelines for Residential Aged Care
• Administer care and treatment as prescribed by Medical Officer and Allied Health Professionals
• Provide ongoing, responsive and holistic care to residents, in consultation with all relevant stakeholders and in accordance with care plans
• Administer and document residents medications and prescribed treatment accurately and in accordance with policy and regulatory requirements
• Manage and administer wound care and monitor progress as per guidelines and UnitingSA policy and procedures
• Respond to any changes in a residents condition appropriately and liaise with GP/Primary Health Practitioner for review
• Maintain accurate clinical documentation and patient information systems ensuring the data is accurate and complete
• Document client medications and prescribed treatment accurately
• Ensure reporting standards are maintained and reviewed to meet clinical audits
• Ensure assessments are carried out effectively, achieving maximum funding in accordance with residents needs

KEY REQUIREMENTS:

• Bachelor of Nursing
• Additional relevant qualifications
• Registration with the Australian Health Practitioner Regulation Agency (AHPRA)
• Current National Police Clearance or DCSI Employment Clearance
• Minimum of 3 years post registration experience
• Experience coordinating clinical care
