Coordinator, Community Aged Care - CHPS / ELS



The opportunity to work in an exciting, innovative and fast paced community aged care environment. Looking for a passionate and experienced Coordinator to facilitate the delivery of personalised high quality care and support for older people living in their own homes.

The successful candidate must be capable of managing a team of support workers, maintain professional boundaries, and possess an optimistic outlook on life and a genuine desire to make a positive difference in the community. Must also excel working under pressure, be detail orientated, and ben an open communicator who values working as part of a team whilst making others feel valued and heard

The Coordinator is responsible for the individualised care, support and services for consumers including the coordination, scheduling and implementation in accordance with approved funding, guidelines, legislative requirements and organisational work instructions.

The successful applicant will manage care and support which enables the consumer to live safely and independently at home while maintaining wellness, safety and connectedness, in line with the consumers co-designed Care Plan and within the scope of practice of the role. The coordinator will work in collaboration with the multidisciplinary team to deliver clinical care (as appropriate) and outcomes for consumers.


  • Work in partnership with consumers, key stakeholders and their carer/advocate to co-design individualised care, support and services for consumers through planning, coordination, scheduling and implementation of personalised and detailed goal orientated Care Plans
  • Regularly make contact with consumers and review their goals, needs and preferences, and update Care Plans with changes
  • Adhere to funding, guidelines, legislation and organisational requirements relevant to the consumer’s program, package or funding
  • Appropriately manage performance and coaching support to direct reports, providing regular informal and formal performance feedback and reviews, supervision and support for relevant training and development
  • Ensure direct reports are aware of and comply with organisational policies, procedures and work instructions

To be successful in the role you will have:

  • Minimum qualification of diploma in Human Services / Health / Community / Disability or equivalent, or extensive experience working in a similar role (essential)
  • Current National Police Clearance – Aged Care (essential)
  • Current First Aid Certificate (desirable)
  • Current SA Drivers Licence (essential)
  • Reliable vehicle and comprehensive vehicle insurance (essential


  • Must be available to work between the regular business hours of 08:00-18:00 AND MUST participate in after hours on call service (shared rotating roster between the hours of 06:00-20:00, 7 days per week).
  • Applicants MUST have community aged care Coordination (or similar) experience in Commonwealth Home Support Program (CHSP) or Consumer Directed Care Home Care Packages (CDC).
  • Experience working with bicultural and bilingual clients will be highly regarded, including group work
  • Multiple permanent positions available – part time (4 days per week minimum) and full time
  • Excellent pay rate (SCHADS SACS 5-6 dependent upon qualifications and experience)
