About the opportunity

An exciting opportunity exists for a Full-time Registered Nurse to join our friendly Home and Community Care Team based in Chatswood, servicing our clients in their home throughout Lower North Shore, Kuring Gai and Northern Beaches.

Working closely with the Clinical Nurse Specialist and other RN in the team, you will be responsible for providing personalised clinical support to seniors that enables self-determination, as well as improving social engagement, meaning and purpose.

Key responsibilities include:

• Carrying out client clinical assessments, referrals and reviews
• Providing clinical communication, supervision and support to the team
• Working collaboratively with the Home and Community Care operational team in developing client care plans
• Monitoring and ordering consumables in consultation with the client, Clinical Nurse Specialist and Support Advisor
• Assessment, treatment and management of wounds within the community

Qualifications

• Current Australian Health Professional Registration Authority for a Registered Nurse
• Experience in a home and community care clinical nursing role (desired)
• Current First Aid Certificate
• Current Australian Drivers Licence
• Experience in Aged Care funding (desired)
