Tuart Place is the leading support service in WA for adults who experienced out-of-home care during childhood, including members of the Stolen Generations; former child migrants; and non-indigenous Australian-born care leavers.

We are committed to providing a trauma-informed service of excellence with a focus on participant (care leaver) leadership and engagement. Participants can access a range of customised clinical services including counselling and therapeutic support groups as well as participate in social activities and our drop-in centre.

ABOUT THIS POSITION

  • Work with older adult Care Leavers - people who lived in institutions or foster care as children, including Forgotten Australians, former child migrants and members of the Stolen Generations to assist them to understand and access My Aged Care services
  • Provide a range of supports including basic information, navigation through My Aged Care website, and support to access services
  • Identify and document any barriers to accessing aged care services
  • Work with a range of stakeholders to develop relationships and referral pathways within the local community
  • Deliver group information sessions to Care Leavers and other stakeholders
  • Share and offer insights with Management Team and interstate counterparts on this jointly funded COTA project

SELECTION CRITERIA

  • Highly developed interpersonal skills and emotional maturity
  • Able to work independently and use sound judgement
  • Understanding of the impact of trauma and possible ramifications such as low level literacy, social isolation or hesitancy engaging with mainstream services
  • Experience in or knowledge of the aged care sector in WA and experience navigating the My Aged Care program
  • Experience capturing and reporting quantitative and qualitative data
  • Relevant tertiary qualification or equivalent competency and experience
  • Proficiency using Office 365 (or similar), Excel, Word and file sharing systems
  • Reliable well-maintained registered and fully insured vehicle
  • Ability to occasionally work from home or an alternative flexible location
  • COVID19 proof of vaccination certificate or approved medical exemption
