Our services create intentional avenues for young people to explore opportunities, build support networks, and access, participate and contribute to their communities. Youth services provide a diverse range of programs which include housing and homelessness, driver training, education, employment and training, AOD support, social and community activities, specialist therapeutic responses and youth justice programs.

THE ROLE

We are currently seeking a values-driven and proactive Youth Services Case Manager to provide a flexible and assertive case management response to clients with multiple and complex needs. Responsibilities include:

  • Ensure that clients with multiple and complex needs within the North West region receive an appropriate service response to assist them to transition out of the homeless system
  • Work collaboratively within Youth Services Integrated Model, having access to all programs and providing case management support
  • Expected to carry a caseload of 12 young people; this includes active participation within our Integrated model
  • A no wrong door approach ensures young people at risk of falling through service gaps are provided with a flexible and responsive approach that reduces risk of further disadvantage and vulnerability
  • Facilitating access for each young person to the support they need, at the first point of contact, and acknowledge that a young person may not choose to work with us or be suited to the programs we offer
  • Work with other Salvation Army expressions, external service providers and partners to ensure a response can be found to address a young person's needs and ensure continued engagement with services
  • Ensure a thorough response, we will follow-up with any external services we have referred to, ensuring support has been provided and determine if further flexible responses are required

With acknowledgment of the valuable and dynamic Integrated service delivery that support sustainable outcomes, we are committed to actively working alongside young people and their identified support systems.

The Intensive case management position covers the North West region of Melbourne and will be based across both the Sunshine and North Fitzroy sites. This role sits alongside the following program areas:

  • Youth Housing West - is the Access Point for the Brimbank and Melton Local Government Areas (LGAs), for people who are homeless or at risk becoming homeless, delivering a continuum of support services including programs with an increasing focus on intervening early to prevent stress in housing and homelessness

The operational hours are 9am-5pm and YS provides assistance to approximately 450 young people and families per annum.

HOURS AND SALARY

This is a permanent full time position, 38 hours per week

Salary and conditions of employment are in accordance with Social, Community, Home Care and Disability Services Industry Award 2010, level 5.

ABOUT YOU

  • Degree qualification in social work or related field
  • Relevant experience in a social service environment is preferred
  • Knowledge of youth homelessness, crisis intervention, and the ability to collaborate with other services within the North West region
  • Demonstrated skills in engaging & working with young people with complex support needs, specialist skills in youth AOD, mental health, out of home care & adolescent violence
  • Assessment skills & knowledge of a range of theories including bonding & attachment, trauma, child and adolescent development & strengths based approaches
  • A current VIC Drivers Licence
  • A Current Working with Children Check
  • Be able to provide proof of Eligibility to Work in Australia
