The Salvation Army Youth Services offer an integrated suite of programs engaging with young people across Australia. Our services create intentional avenues for young people to explore opportunities, build support networks, and access, participate and contribute to their communities. Youth services provide a diverse range of programs which include housing and homelessness, driver training, education, employment and training, AOD support, social and community activities, specialist therapeutic responses and youth justice programs.

THE ROLE

We are seeking a values-driven and proactive Youth Specialist North/West Metro Case Manager to coordinate all secondary consults for clients of the North West Metro service.

Responsibilities include:

  • To deliver flexible and assertive case management response to clients with professional and consistent case management to those experiencing homelessness or at risk of experiencing homelessness.
  • Ensure that young people within the North West region receive an appropriate service response that upholds The Salvation Army's Model of Care, Journey Towards Independence.
  • Work collaboratively within Youth Services Integrated Model, having access to all programs. With acknowledgment of the value that an Integrated service delivery model has in supporting sustainable outcomes.
  • Demonstrate commitment in actively working alongside young people and their identified support needs
  • Coordinate Referrals and intake of all clients across all.
  • Hold Interim Response clients via the access point.
  • Manage all active Holds for Better Futures Clients and engage in active assessments of young people.
  • Work in a manner consistent to the expectations of the Journey to Independence Model and that follows the framework of a Psychologically Informed Environment.
  • Complete comprehensive intake and risk assessment for all young people, incorporating adequate assessment of the needs of accompanying children where relevant.
  • Work to the direction of case management plans in assisting young people to attend appointments relating to health & wellbeing, sustainable housing, education & employment, life & living skills and social inclusion & participation.
  • Assist young people with a range of activities in preparing for independent living
  • Provide case management support to a caseload of young people who are experiencing high and complex needs
  • Participate in duty rosters and actively manage youth entry point
  • Work alongside the youth services team at the Sunshine youth hub to provide an assertive response to young people presenting in crisis

HOURS AND SALARY

This is a permanent full time position, 38 hours per week.

Salary and conditions of employment are in accordance with Social, Community, Home Care and Disability Services Industry Award 2010, level 5.

ABOUT YOU

  • Tertiary qualification in social sciences, welfare or related field with a degree qualification preferred. Relevant experience in a social service environment
  • Strong verbal and communication skills
  • A sound understanding of theories relating to trauma and attachment as they relate to service delivery for young people who have suffered neglect and abuse, both on an individual level as well as within the broader refuge environment
  • Understands and identifies indicators of physical, emotional, psychological or sexual abuse in infants, children and adolescents
  • Knowledge of youth homelessness, crisis intervention and the ability to collaborate with other program areas
  • Skills in engaging with the specified client group; demonstrated capacity to build effective relationships with clients based on social work, youth work and psychologically informed environments
  • A current VIC Drivers Licence
  • Valid VIC Working with Children Check
  • Ability to provide proof of Eligibility to Work in Australia
