Access Health aims to provide a non-judgmental, responsive and comprehensive Primary Health Care Service responding to the diverse health and other needs of people experiencing disadvantage related to (but not limited to) homelessness, drug use, sex work, mental health and forensic history. Located in St Kilda, Victoria the Access Health hub offers a range of services such as harm reduction, addiction medicine, mental health, sexual health, general health, counselling, groups, outreach and health promotion. Services are provided via partnership with numerous partner organisations. We recognize the immense resilience, strength and abilities of all individuals and seek to harness this for the achievement of positive outcomes.
This role will work alongside a team of Nurses and Social Workers undertaking a broad range of tasks at the front line of services. High level administration, computer and communication skills are required as a significant proportion of work is related to the intake, coordination and exit of patients visiting on-site clinicians on a day to day basis (managing waiting rooms/triage, creating patient files, Medicare billing). In addition, front of house Duty Social Workers undertake a broad range of other activities such as client engagement and support, brief interventions, assessment and referral, service coordination, system navigation and provision of health, mental health, AOD and harm reduction information and education.
Access Health are seeking a dynamic, part time - 2 days per week, social worker to join a multidisciplinary team of AOD, mental health and health practitioners. This role will form part of the Access Health Specialist AOD and Harm Reduction Primary Health Care team, supporting the operation of all front of house activities including client engagement, intake, brief interventions, assessment and referral, service coordination, system navigation and provision of health, mental health, AOD and harm reduction information. A proportion of this work is managing medical reception duties such as Medicare triage, answering phones and medicare billing. The Duty Social Worker role at Access Health is diverse and interesting, holding the space as the main point of contact and communication for all people accessing the service and all practitioners working within the service.
