About the Role

We are seeking a motivated and dedicated Support Coordinator for our NDIS program based at James Barker House in Footscray.

The NDIS Support Coordinator, is a leader in the disability community care team, and has supervisory responsibility for a small team of NDIS Support Workers. The key responsibilities include providing assistance to strengthen participant's abilities to connect and participate more fully in their community, to achieve their goals, and to live a meaningful life. The position will provide assistance with on boarding and linking participants with the right services to meet their needs. Coordinating a range of both funded and mainstream supports and helping to build on informal supports. Working with participants to provide timely and individualised services, resolve points of crisis and work to develop resilience in their own network and community. To support The Salvation Army in continual process and service delivery improvement to meet participant needs within NDIS service provision.

You will strive to build strong and positive relationships with all stakeholders and deliver services with compassion, integrity and dignity.

You will have

  • An appreciation of and the ability and willingness to promote and support the values and ethos of The Salvation Army
  • Have a demonstrated understanding of the NDIS and experience in coordinating and supporting participants to implement their NDIS Plans
  • Experience working with people with a disability and their support networks
  • Minimum Certificate III or IV in Disability/Mental Health or similar
  • Computer literacy and experience with documentation and reporting
  • Excellent communication skills and ability to collaborate and liaise with all stakeholders
  • Ability to work independently and lead a team of support workers
  • A current unrestricted driver's licence and access to a car
  • A current First Aid Certificate

In addition, you will be well supported by a passionate team and be provided with ongoing training and education, and have access to generous salary packaging arrangements.

The successful candidate will also be required to clear probity checks including a National Criminal History Record Check and Working with Children Check prior to commencing employment.
