About the role

To provide psychosocial support and case management to clients accessing transitional accommodation following exit from Barwon Health acute mental health services.

  • Assess clients suitability for the mental health transitional housing program
  • Support clients in their move to transitional accommodation and participation in organised program activities
  • Case manage clients including the development and review of individual case plans and provide interventions as identified
  • Assist clients to explore long term accommodation options and support the transition into new accommodation
  • Work within a framework which fosters empowerment and encourages active participation in the community
  • Provide activities that encourages clients to identify and build on their strengths and abilities and to support them to work towards their goals in a supportive and flexible manner
  • Facilitate house meetings and other group activities
  • Maintain client case files and accurate client data collection
  • Minimum of 2 years experience in the Mental Health sector
  • Experience in the homelessness sector desirable
  • A national police record check is required
  • A current valid Working with Children Check
  • A current Victorian Driver's license
  • Tertiary qualification in welfare, social work, psychology or other relevant discipline
