The Salvation Army Australia's Homelessness Stream provides high quality, person centred services to people at risk, and or experiencing homelessness. We support adults and families, including accompanying children with accommodation, case management, outreach support, financial assistance, connection and referral to other specialist services.

The Housing Pathways Program (HPP) sits within the Western Metro Homelessness Service and provides housing services and support to connect with the housing system for those in custodial care in the metropolitan Melbourne and Barwon region of Victoria; specifically, the Melbourne Assessment Prison, the Metropolitan Remand Centre, Port Philip Prison, the Judy Lazarus Transition Centre, Barwon Prison and Marngoneet Correctional Centre (Karreenga).

THE ROLE

We are currently seeking a values-driven and proactive Enhanced Housing Pathways Worker to assist people (both men and women) being released from any prison within Victoria. Responsibilities include:

  • Clients exiting prison will be connected to housing and support options, as assessed by prison based initial assessment and planning workers.
  • Provide clients with a capability and needs / private rental sustainability assessment.
  • Support clients to develop an individual case plan and the journey towards goals will be monitored and reviewed.
  • Assist clients to access housing relevant to their needs (emergency, transitional, private rental, social and supportive housing).
  • Assist clients to access support services and to establish meaningful community supports.
  • Clients (women) who have experienced family violence, or are at risk of family violence, are assisted to access flexible support packages.
  • Clients with complex needs who are at imminent risk of homelessness upon release are provided with enhanced interventions and integrated responses

Reporting to the Team Leader, Housing Pathways Program this position is an outreach role, based out of the HPP office, situated at the Western Metro Homelessness Service in Sunshine.

HOURS AND SALARY

This is a full time position, number of hours per week fixed term to 30 June 2023.

Salary and conditions of employment are in accordance with Social, Community, Home Care and Disability Services Industry Award 2010, level 5.

SUCCESSFUL APPLICANT WILL HAVE:

  • Demonstrates knowledge of homelessness including context, causes, issues, philosophies and legislation.
  • Understands and applies homelessness models, approaches, theories and practices when dealing with clients.
  • Ability to understand and respond to the needs of individuals experiencing homelessness or at risk of homelessness.
  • Whilst not essential, a Tertiary qualification in Social Work or related community service field will be favourably regarded.
  • Experience working within correctional settings is considered highly desirable.
  • A current VIC Drivers Licence
  • Valid VIC Employee Working with Children Check
  • Ability to provide proof of Eligibility to Work in Australia
