Our programs and services equip people with the tools they need to live a positive and healthy lifestyle.

ABOUT THE ROLE

We are currently seeking a values-driven and proactive Creating Connections worker to provide support and assistance to young people aged 15-25 who are homeless or at risk of homelessness to maintain meaningful connections to their community, build on existing strengths and develop self-reliance.

Other key responsibilities include:

  • Assessment of client needs within established case plan
  • Managing and assessing the risk of inter relationships within environment, de-escalation of situations
  • Progress and outcomes of case plans are monitored and actively engaged with
  • Effective relationships are established with clients that encourage self-determination and participation
  • Work to build a strong rapport with young people using creative and flexible and respectful approaches
  • Build clear and strong boundaries for service provision to ensure client safety.
  • Advocacy on behalf of, or with, clients occurs where required to ensure access to essential items services and resources

This position is located at 212 Morton St Mt Pleasant with weekly contact at 6 Crompton St Soldiers Hill.

HOURS AND SALARY

This is a part time position, at 15 hours per week, ongoing. Days of work are flexible and negotiable.

Salary and conditions of employment are in accordance with Social, Community, Home Care and Disability Services Industry Award 2010, Social Work Stream, Level 5.

ABOUT YOU

  • Tertiary qualification in Youth Work / Social Work or related field with a Degree qualification preferred.
  • Minimum 3 years experience in relevant experience in social service environment
  • Experience and resilience in working with complex clients presenting with challenging behaviours
  • Shows commitment to achieving challenging and complex goals
  • Builds culture of consultation and works proactively to overcome barriers to collaboration
  • Acknowledgement and responsiveness to diversity of experiences, perspectives, values and beliefs
  • A current Victorian Drivers Licence is essential
  • Be able to provide proof of Eligibility to Work in Australia
