About the role

A rare opportunity exists for an experienced Registered Nurse in aged care, with strong leadership skills and a passion for excellence in care, to relocate to beautiful Mossman QLD 4873. Great lifestyle change, offering calm environment and away from the bustling city Our Clinical Coordinators are responsible for providing clinical leadership, guidance, supervision and education to all staff to promote excellence in care.

We are seeking an experienced Registered Nurse with leadership experience in Aged Care, to join the team at our boutique 42-bed Kubirri Aged Care Centre located in Mossman QLD 4873 in the role of the Clinical Coordinator. This role is available on a permanent full-time basis.

Located in the beautiful town of Mossman, The Kubirri Aged Care Centre will provide the best of both worlds. The Centre will have a tranquil backdrop of the Mossman River, views of open farmlands and yet is only a couple of minutes away from the Mossman town centre, hospital and local attractions. Designed in consultation with the community, industry experts and peers, our 42-bed home will offer a warm, inviting community atmosphere aimed to create an enhanced living experience for older Australians in the area. The Centre also leads the way with a sustainable design to minimise our environmental footprint.

Kubirri Aged Care Centre will offer permanent residential care, including dementia care, palliative care and respite (short-term) care.

ABOUT THE ROLE

As the Clinical Co-ordinator you will be responsible for coordinating the delivery of safe, effective and individualised care to residents and their families (including managing staff and physical resources) and assisting residents to lead an active and meaningful life as independently as possible in keeping with the organisation's Mission, Vision and Values. You will work with and assist the Centre Manager to plan, coordinate and evaluate the clinical care in the centre to maintain full accreditation and achieve best practice. The main responsibilities of the role are, but not limited to:

  • To provide clinical leadership in the coordination of nursing and care services
  • To promote and support a person-centred culture and values - respect and treat all people as unique individuals with needs, preferences and a past history
  • To optimise ACFI funding in relation to resident care plans, and ensure that sufficient evidence is maintained for validation of all claims
  • To maintain accountability and responsibility for the clinical management of health and care outcomes of the residents in the Centre
  • To ensure that skills and knowledge appropriate to delegate the task/authority has been considered when delegating health and care responsibilities to other professionals
  • To exercise critical thinking within an evidence-based framework to ensure the delivery of high-quality health and care services which promote optimal health outcomes for the residents

ABOUT YOU

To be successful in this role you will have:

  • An understanding of, and empathy with, the Christian faith, values and ethos of The Salvation Army
  • Registered Nurse - Current unrestricted registration with AHPRA
  • Minimum four years of clinical experience as a Registered Nurse (preferably in Aged Care)
  • Demonstrated knowledge in the care needs of the older person
  • Understanding of and commitment to continuous quality improvement
  • An understanding of and demonstrated experience with Aged Care Quality Standards and relevant Aged Care legislation
  • An understanding of and demonstrated experience with Aged Care Funding Instrument (ACFI)
  • Demonstrated excellent organisational and time management skills
  • High level verbal and written communication skills
  • You must be a self-starter and able to work autonomously
  • Strong IT skills and experience in aged care clinical management using iCare
