Salvation Army Youth Family Services is committed to working in partnership with young people aged 16-25 to address the issues contributing to their unique experience of crisis & homelessness. The program is operated under the framework of a Psychologically Informed Environment, with a particular focus on assisting young people to:

  • Develop life and living skills to prepare for independence
  • Address issues in relation to health & wellbeing
  • Promote opportunities to overcome exclusion and isolation
  • Explore education pathways and participation
  • Work towards long term sustainable housing

We are seeking an enthusiastic and experienced Case Manager to deliver high-quality case management support services to young men, women, and families, including those with accompanying children who are at risk of or experiencing homelessness.

This is a fixed term 6 month full-time position, 38 hours per week based in St Kilda at the Upton Road Youth Refuge.

Applications will only be assessed if they include CV and cover letter addressing the following:

  • Build effective relationships with clients and encourage self-determination and participation
  • Provide ongoing assessment of the client's issues, needs and circumstances from initial intake to exit
  • Develop individual case plans in collaboration with clients to meet a range of interventions needs
  • Assist clients to access support services and establish community supports
  • Regularly monitor and review client's individual case plans meeting needs or goals
  • Develop implement and review therapeutic case plans with the Client Services manager
  • Maintain accurate and up to date client records using TSA information systems
  • Engage and work with young people with complex support needs

You will have

  • Relevant Bachelor or higher qualification in welfare or social work is required
  • Demonstrated experience and highly developed skills in working with people who have multiple and complex needs
  • Experience in working with young people who have complex trauma and attachment injuries
  • Be proactive and self-motivated
  • Be able to convey information effectively – verbally and in writing
  • A current Victorian Drivers License is essential
  • A current and valid Working with Children's check
  • Willingness to undergo a National Police check

The Salvation Army is subject to public health orders. You may be required to provide evidence of vaccine status dependent on requirements.
