The Salvation Army Outreach Connections Program operates a community based, assertive outreach case management model to identify and engage with those who have complex and multiple needs, experienced recurring homelessness, and who have barriers to accessing/engaging with mainstream and specialist services. The service objective is to promote appropriate housing outcomes, improved health & well-being and social connectedness that lead to quality of life outcomes.
This position provides case management support to adults (18+) in the Lower Hume Region working with six other Team Members based in Wangaratta, Seymour and Shepparton. The primary purpose of the role is to:
A relevant Tertiary qualification in welfare or social work is desirable. Previous experience working is the homelessness sector will be highly regarded
