The Salvation Army Outreach Connections Program operates a community based, assertive outreach case management model to identify and engage with those who have complex and multiple needs, experienced recurring homelessness, and who have barriers to accessing/engaging with mainstream and specialist services. The service objective is to promote appropriate housing outcomes, improved health & well-being and social connectedness that lead to quality of life outcomes.

ABOUT THE ROLE

This position provides case management support to adults (18+) in the Lower Hume Region working with six other Team Members based in Wangaratta, Seymour and Shepparton. The primary purpose of the role is to:

  • Provide, as part of a team, Case Management support and Assertive Outreach to individuals and families who are homeless or at risk of becoming homeless with multiple and complex needs.
  • Engage with clients in outlying areas (involves travel) who are homeless, with the aim of supporting them in relation to housing needs and access to mainstream services.
  • Provide information, advocacy and linkages into community supports and programs for clients who are homeless or at risk of homelessness with multiple and complex needs.
  • Work collaboratively within the broader Programs of the Salvation Army in order to provide an integrated support response to the target group.

ABOUT YOU

A relevant Tertiary qualification in welfare or social work is desirable. Previous experience working is the homelessness sector will be highly regarded
