The Salvation Army Australia's Homelessness Stream provides high quality, person centred services to people at risk, and or experiencing homelessness. We support adults and families, including accompanying children with accommodation, case management, outreach support, financial assistance, connection and referral to other specialist services.

Our philosophy is that every person deserves dignity, respect and quality of service and that no one should be without a safe, affordable and secure home. Services are free, voluntary, accredited and delivered by professional and committed staff.

We are currently seeking a values-driven and proactive Case Manager to provide case management to people who are homeless or at risk of homelessness.

You will successfully

  • Develop holistic individual case plans that encompass a range of strength-based and solutions-focused interventions in collaboration with clients to address their needs and goals
  • Assist clients to access support services and establish community supports, and where needed are assisted through advocacy with service providers
  • Coordinate supports through case management practices, and referral to key support agencies
  • Conduct Outreach to clients in accommodation
  • Provide expert advice to both clients and employees regarding homelessness
  • Develop, plan, and supervise the implementation of case plans including employment/ educational and developmental living skills

Located in Geelong and reporting to the Support Services Team Leader, this is a fixed term full time position for 12 months.

Salary and conditions of employment are in accordance with Social, Community, Home Care and Disability Services Industry Award 2010, level 5.

You will have

  • Social Work qualification or other relevant social tertiary qualifications and/or substantial experience
  • Experienced working with women and accompanying children experiencing family violence
  • A current VIC Drivers Licence
  • Valid VIC Employee Working with Children Check
  • Ability to provide proof of Eligibility to Work in Australia

The Salvation Army is subject to public health orders. You may be required to provide evidence of vaccine status dependent on requirements.
