ABOUT ROLE

  • To support young people and young families to achieve housing stability, experience improved social inclusion and be equipped to live successfully in the community. This support is achieved through intentional case management activities and working effectively with other members of a multi-disciplinary team to provide a comprehensive client service within the values and mission of TSA.
  • To offer appropriate support to young people and young families who are homeless or at risk of homelessness within an intentional Case Management framework, including referral and assessment, advocacy, relationship building, development of community support and the development of case plans. Maintain case management documentation, records, reports and data collection.
  • Acquire and maintain a good working knowledge of accommodation and support options for homeless people both within TSA Network and external agencies and work collaboratively and professionally with other agencies.
  • The Salvation Army Youth Program operates from a Trauma Informed Care framework and this position supports the restoration of choice, empowerment and safety for at-risk young people. The case manager's role requires the successful candidate to work effectively and collaboratively in a team environment. Record and maintain accurate client records within TSA policy.

ABOUT YOU

  • Solid level of experience (minimum 2+ years) in the implementation of case management and systems for disadvantaged and/or complex needs clients.
  • Experience working with young people from culturally and linguistically diverse backgrounds and religions.
  • Experience working in collaboration with other organisations to develop and tailor programs to meet clients' needs.
  • Desirable that the successful applicants have an understanding of the issues faced by people experiencing homelessness, substance use disorders, mental ill health and other disabilities.
  • Tertiary or other qualifications within Community Services, Social Work or similar discipline, or extensive relevant experience with a minimum of Certificate IV qualifications
  • Professional development required to maintain skills for the role.
  • Current First Aid Certificate.
  • Current Driver's licence.
  • Working with children check (blue card) or ability to obtain.
  • National police check.
