To provide intake, assessment, case management and support to clients who access the Red Shield Hostel for crisis accommodation.

Client Assessment and Support

  • Conduct assessments using appropriate information, tools and techniques to gather and analyse information.
  • Relate to people in a manner which is appropriate to their developmental and cognitive level of functioning and provide services in a professional and friendly manner.
  • Use appropriate risk models to assess levels of risk, harm and well-being, including complex risk assessments and the ability to action a plan to mitigate any identified risk. High risk resident must be promptly reported to the supervisor for appropriate action.
  • Facilitate and monitor the case management of clients, including active participation in case review meetings.
  • Participate in the access and intake system, answering phone calls and enquiries, screening and assessing needs, prioritising and making appropriate follow up appointments and referrals.
  • Maintain up-to-date client records and comply with data collection standards.
  • Positive working relationship clients and other key stakeholders.
  • Positive working relationship with clients seen through joint achievement of goals and client engagement.
  • Clients receive adequate levels of support in achieving goals and transitioning to independence.
  • Clients referred and connected to services, community and support.
  • Progress and outcomes of individual case plans are regularly monitored and reviewed and where needed action is taken to ensure the plan continues to meet client needs or goals.
  • Clients appropriately supported to transition into alternative accommodation

Building Relationships & Engagement

  • As one of the first contact point for people accessing the centre, the case manager will need to build a positive rapport with a wide range of people, as well as their family and friends.
  • Respects client confidentiality and maintain appropriate professional boundaries.
  • Build and maintain effective relationships with key stakeholders and assist other stakeholders to provide holistic and integrated care.
  • Relates to and communicates with people from diverse 'cultures', locations, ages and backgrounds.
  • Remains calm in difficult or aggressive situations.

Team Participation

  • Works collaboratively and co-operatively with other staff.
  • Attend and actively participate in team meetings.
  • Works cooperatively with others both formally and informally within the team.
  • Shared goals are developed and accomplished by taking an active role in the team and contributing ideas.
  • Accepts responsibility for accomplishing shared goals.

Administration

  • Risks to clients, staff and TSA are identified and bought to the attention of management as soon as possible.
  • Communicates effectively through thorough and accurate recordings including case notes, log book, diary and other reporting methods.
  • Participate in regular client reviews.
  • Prepare written reports, as required.

Professional Development

  • Engage in regular supervision.
  • Maintain knowledge of relevant sectors.
  • Sound working knowledge of principles and practices in areas including trauma informed practice and mental health.
  • Participate in professional development, training and forums, as required.

SELECTION CRITERIA

Values

Required experience

  • Extensive, demonstrated previous experience working in a social service environment.

Qualifications & Mandatory Requirements

  • Tertiary qualification in health, behavioural/social sciences, community services, or a related discipline that provides a sound understanding and knowledge of assessing and working with people at risk. A degree qualification is preferred.
  • NT Working with Children Check.
  • National Police check.
  • A current First Aid certificate.
  • A current drivers licence.
