The Salvation Army Australia's Homelessness Stream provides high quality, person centred services to people at risk, and or experiencing homelessness. We support adults and families, including accompanying children with accommodation, case management, outreach support, financial assistance, connection and referral to other specialist services.

The Housing Connect-SASH program operates within the Tasmanian Division of The Salvation Army Southern Territory (TSA). SASH is part of the Housing Connect collaborative partnership and provides flexible, specialist support to people who are homeless, at risk of homelessness or in housing need .

THE ROLE

We are seeking a values-driven Case Manager to work with people who are homeless or at risk of homelessness, to develop and implement case plans that will facilitate their transition to independent living.

The successful applicant will provide information, referral, advocacy, specialist homelessness support and links service users to a range of services in order to enhance their housing options, physical and mental well-being, quality of life and independent status in the community.

HOURS AND SALARY

This is a full time position, 38 per week fixed term to June 2021.

Salary and conditions of employment are in accordance with Social, Community, Home Care and Disability Services Industry Award 2010, level 5.

SUCCESSFUL APPLICANT WILL HAVE

  • Tertiary qualification in social work, community welfare, community development or a related field, a degree qualification is preferred
  • Relevant experience in a homeless, outreach service environment is required. Highly developed knowledge of homelessness and the social housing service system, particular the homeless accommodation sector
  • Demonstrated experience and expertise in working within a psychosocial framework with homeless clients who may have complex mental health, health and. or addiction issues, and intellectual and physical disabilities
  • Highly developed case management , case work and case planning skills
