The Community Care Program, aligned with our Aged Care division, provides care and support services to people across Australia to enable them to live in their own home as they age, and to participate in their own communities.
As a Care Coordinator in our Melbourne Community Care Team you will be responsible for the ongoing coordination and management of services and supports to our Community Clients.
You will support, supervise and mentor a small team of care workers to ensure the provision of high-quality services to consumers, that assists them to stay safely at home and within their own community.
We provide home based services primarily to the aged, specialising in caring for those who are socially and financially disadvantaged and we assist consumers with activities of everyday living (personal care, domestic assistance, shopping, meals etc).
