We are currently seeking a values-driven and dedicated Care and Recovery Coordination and Mental Health Case Manager. This position is a dual-purpose role, responsible for providing care and recovery coordination (CRC) through a range of interventions including case management to assist in improving the health and wellbeing for individuals, family members and significant others experiencing alcohol and other drug related harms referred to Barwon AOD & Mental Health Services within The Salvation Army.
In addition, this role will also provide support and case management to clients in the Accommodation & Psychosupport Program (APPS) who are the accessing transitional accommodation following exit from Barwon Health acute mental health services and provide mental health psychosocial secondary consultation to TSA Services in the Barwon region.
The Salvation Army is subject to public health orders. You may be required to provide evidence of vaccine status dependent on requirements.
