THE ROLE

The AOD Practitioner position provides an entry point to AOD services for people with AOD issues who reside in the Inner North / North West, through assessing client need, identifying treatment options and creating holistic treatment pathways. This also includes providing interim support to clients who are awaiting assessment and treatment commencement.

This Position provides liaison, support, case management and a range of interventions to assist in improving the health & wellbeing for individuals, family members and significant others experiencing alcohol and other drug related harms to clients referred for Care & recovery Coordination and Counselling to The STAR AOD program in Inner North / North West of Melbourne.

The successful applicant will provide intervention through exploration of the impact of substance use on the individual, their families and significant others and care and recovery coordination through coordinating care and support around issues caused by substance use. The position:

  • Works with clients from a holistic perspective to facilitate change and positive skills development.
  • Engages with families and significant others to develop family inclusive practice
  • Works as part of a team along with case managers, support workers and other AOD practitioners to achieve the best outcomes for the clients
  • Initiates case plans with client and primary worker to ensure necessary and relevant follow up care is provided
  • Advocates on behalf of clients to ensure access to relevant services.

Reporting to the Clinical Coordinator STAR Program this position is located in Melton.

HOURS AND SALARY

This is a permanent full-time position, 38 hours per week.

Salary and conditions of employment are in accordance with Social, Community, Home Care and Disability Services Industry Award 2010, level 5.

SUCCESSFUL APPLICANT WILL HAVE

  • Relevant qualification in social sciences or other relevant discipline or equivalent experience in a social program environment
  • Understanding of and commitment to the community services sector
  • Experience in basic research and analysis and preparation of documentation
  • Solid knowledge and capability with technology including data bases
  • A current VIC Drivers Licence
  • Be able to provide proof of Eligibility to Work in Australia
