Located in Maroubra, Pathways Maroubra offers community-based programs and referral to residential treatment options working within a harm minimisation model.

Our service provides:

  • Community-Based treatment options
  • Comprehensive assessment, individualised treatment/recovery plans and case management
  • Holistic treatment service that focus on physical, mental, emotional, social and spiritual wellbeing, individualised 1-1 sessions, facilitated groups and living skills training

THE ROLE

We are currently seeking a values-driven and proactive Case Workers to support clients through the process of recovery:

  • Provide harm reduction/minimisation strategies to support clients to achieve their desired outcome.
  • Provide information of all AOD treatment options available and make referrals accordingly.
  • Provide efficient day to day service delivery through education, psycho-social interventions, group work, assessment, referral and case work within a recovery-orientated and person-led service environment.
  • Develop, implement and monitor strengths-based case plans, including exit planning
  • Ensure a safe environment where risks are identified, assessed and managed
  • Establish effective relationships with clients that encourage self-determination and participation
  • Strong referral pathways are built and maintained with internal and external stakeholders

HOURS AND SALARY

This is a fulltime position, fixed term. Continuation of this role after 30 June 2021 is dependant on funding.

Salary and conditions of employment are in accordance with Social, Community, Home Care and Disability Services Industry Award 2010, Level 4

IDEAL CANDIDATE WILL MEET THE FOLLOWING SELECTION CRITERIA

  • Knowledge and experience of working within AOD and/or mental health setting
  • Experience of assessment and care planning
  • Good understanding of risk management
  • Experience working with complex needs/co-morbidities
  • Minimum Diploma level qualification in a relevant discipline (AOD, Community Services, Case Management) or including core AOD Units (or willing to undertake/complete during employment).
  • High level of interpersonal skills including the ability to consult and negotiate with a range of internal and external stakeholders.
  • Good planning, organisational, problem solving and evaluation skills.
  • Ability to work with people of diverse backgrounds and experiences
  • A current NSW Drivers Licence
  • Be able to provide proof of Eligibility to Work in Australia
