The Bridge Program provides a holistic stepped care approach to treatment and offers clients choice of residential rehabilitation, day programs and outreach services. The structured program (Residential and Day) operates for 10 weeks and includes psycho-educational groups, case management, a range of evidence based therapies and modalities of treatment.

THE ROLE

We are currently seeking a values-driven and proactive Support Worker to provide case management support to clients throughout their stay at The Bridge. Other key responsibilities include:

  • Provide after-hours support for residents of the Bridge Program
  • Support a team of case workers by ensuring residents comply with policy and procedures of the program after hours
  • Assess client issues, needs and circumstances and report these to the case facilitator team
  • Effective professional relationships are established with clients that encourage self-determination and participation.
  • Accurate up to date client records and data is are maintained for all clients according to the TSA standards and procedures.
  • Risks to clients , staff and TSA are identified and brought to the attention to the Team Leader as soon as possible.
  • Appropriate preventative action is taken in relation to potential high risk situations.

This position is located in Ulverstone.

HOURS AND SALARY

This is a casual position.

Salary and conditions of employment are in accordance with Social, Community, Home Care and Disability Services Industry Award 2010, level 3.

IDEAL CANDIDATE WILL HAVE

  • Certificate IV level qualification in relevant field e.g. AOD or Mental Health or related field and relevant experience
  • Experience and resilience in working with complex clients presenting with challenging behaviours
  • Ability to encourage and cooperate with others to achieve common goals
  • Acknowledgement and responsiveness to diversity of experiences, perspectives, values and beliefs
  • A current TAS Drivers Licence
  • A current and valid First Aid Certificate
  • Be able to provide proof of Eligibility to Work in Australia
