About the Role

We are looking for a passionate and driven individual to join us as a Care Coordinator. In this full-time role (8:30 am to 5:00 pm), you'll manage a high volume of care recipients, ensuring timely organisation of support services, ongoing customer support, and budget management. You will work from our young and vibrant Bowen Hills office, receiving fully paid on-the-job training. No industry experience? No problem! We’ll equip you with everything you need to succeed.

Job Description

  • Be the trusted point of contact for your portfolio of care recipients, ensuring ongoing communication and support.
  • Update and maintain consumer records, keeping detailed case notes.
  • Coordinate care recipient care, including:
  • Scheduling care and support workers for numerous care recipients.
  • Managing and monitoring care recipient budgets to prevent overspending.
  • Sourcing and managing information to support care recipients in achieving their care goals.

Skills and Qualifications

  • Excellent communication and customer service skills.
  • Coordination experience (highly desirable).
  • Service-oriented with strong interpersonal skills.
  • Proficient computer skills with knowledge of Microsoft 365.
  • Well-organised, able to prioritise and meet deadlines.
  • Detail-oriented, problem-solver, and decision-maker.
  • Enjoys working in a multi-disciplinary team.

Mandatory Requirements

  • Current federal police clearance (issued within the last 12 months).
  • Valid Australian work rights (e.g., passport, birth certificate)
