Community Home Support Program (CHSP) Service Coordinator

We are currently recruiting for CHSP Service Coordinator, you will be responsible for ensuring the provision of high-quality services to our CHSP clients. You will develop relationships with CHSP through annual reviews and service coordination.

Key duties:

Primary Objectives

  • Ensure that the delivery of the program is in accordance with the organisations policy and procedure, applicable legislation, standards and guidelines.
  • Ensure client documentation is up to date
  • Provide information to clients about Aged Care services we can provide
  • All required data is entered into the client management system to the highest standard of accuracy, and meet payroll, finance, and other relevant deadlines.
  • Apply ethical decision-making processes according to relevant professional and program standards to effectively manage complex consumer issues that involve competing demands or principles.
  • Makes all of the CHSP contact calls throughout the year
  • Completes the annual review for all CHSP clients within their region
  • Monitors MAC and identifies HCP waitlist for all CHSP clients
  • Intensifies phone calls with CHSP clients who have a waitlist of 3-6 months
  • Identifies CHSP clients who require a MAC referral for HCP
  • Initiates and completes MAC referral for HCP in conjunction with client input

You will have:

  • Tertiary qualification in Health or a related field and relevant experience in case management evidenced.
  • Excellent written, verbal and numerical communication skills.
  • Good computer skills, with competence in MS Office and Client Management applications.
  • Experience in assessment and development of care plans.
  • An understanding of Aged Care Reforms and implications for the organisation and consumers.
  • Demonstrated commitment to high quality customer service.
  • Sound networking skills.
  • Ability to work autonomously, sound time management skills and ability to prioritise to meet competing demands.
  • Demonstrated ability to respond positively to change.
  • A current Victorian driver's licence.
  • Satisfactory completion of a National Police Check.
  • Current Working With Children Check.
  • Demonstrated commitment to cultural diversity, equal employment opportunity and workplace health and safety appropriate to the level of the appointment.
  • Demonstrated awareness of the organisation's vision, mission and values

You will also be willing to undergo police and Working with Children Checks, as well as providing 2 recent managerial references.
