Support Coordinator role

The Support Coordinator plays an integral role for NDIS participants in implementing their NDIS plan. In this role you will be assisting NDIA participants and their families to connect to and coordinate informal, mainstream and funded supports in a complex service delivery environment and to report on participant goals and outcomes. It may also include resolving points of crisis, developing capacity and resilience in a participant’s network and coordinating supports from a range of sources.

This role would cover Northern Sydney and surrounding regions. We offer flexible working hours and access to fringe benefits.

To be considered for this role you will need to demonstrate:

  • Demonstrated experience as a NDIS support coordinator or experience of service coordinator or case management in the disability sector
  • Strong understanding and working knowledge of the NDIS operational environment
  • Excellent people skills including the ability to work with clients, families, service providers and various stakeholders.
  • Ability to work effectively under pressure and be able to prioritise tasks with competing priorities
  • Excellent organisational and time management skills
  • Excellent communication skills, written and verbal
  • Demonstrated commitment to and ability in enhancing community participation, skill development and rights of people with a disability.
  • Demonstrated ability to self-motivate and work autonomously and as part of a team
  • Relevant IT skills to ensure adequate skill in record keeping, report writing, NDIS portal access and information interpretation.
  • A current drivers licence and reliable transport
  • A current National Police check
  • Completion of NDIS worker Orientation Module
