Team Leader - Support Coordination

TeamHEALTH has become the leading not for profit provider of mental health services in the Top End of the Northern Territory. TeamHEALTH has provided outreach services, support and advocacy for Territorians with mental illness for over 30 years.

The National Disability Insurance Scheme (NDIS) is a new way of providing individualised support for people with disability, their families and carers. The NDIS provides Australians with a permanent and significant disability, with the reasonable and necessary supports they need to live an ordinary life. Coordination of supports for NDIS participants is an important aspect in ensuring that participants are empowered to select their preferred provider and to develop an appropriate service agreement.

Reporting to the Business Services Manager the Team Leader, Support Coordination will take a lead role in ensuring that TeamHEALTH’s support coordination service provides participants choice and control, is person centred and recovery focused.

The Team Leader, Support Coordination will be the subject matter expert on NDIS Support Coordination in TeamHEALTH and will work closely with the Business Services Manager to ensure that planning informs, streamlines and transitions TeamHEALTH to become leaders in NDIS Support Coordination in the Territory. They will also champion the provision of support to participants, their families and carers in identifying and connecting with appropriate providers to support participants in the achievement of the objectives of their NDIS Plan.

Possessing qualifications in Mental Health, Disability Support or related area, the successful applicant will have a demonstrated experience in the management and delivery of quality support coordination services and knowledge of the NDIS, proven organisational and communication skills and skilled problem solver.

The successful applicant will be required to have a current National Police Certificate, Working with Children Clearance, NT Driver’s Licence and completed the NDIS Worker Orientation Module.
