You will engage and support young parents and their infant in the transition to parenthood through case management, education and advocacy. You will undertake an initial assessment of family strengths and needs to develop individualised case plans that build on the family’s current strengths and increase resilience to enable them to reduce risk and increase safety and wellbeing. We work in collaboration with other services to provide holistic services that help families achieve their goals.
