We are seeking an experienced, enthusiastic, motivated health professional to be part of a pilot service auspiced by St Vincent's Hospital Melbourne and the Eastern Melbourne Primary Health Network. The Mental Health and Primary Care Partnership Program (MaP) aims to deliver mental health care to older adults with mild to moderate mental illness living in Boroondara.

The St Vincent's Aged Person's Mental Health Service comprises an inpatient service (Normanby Unit), community outreach service (Aged Psychiatry Assessment and Treatment Team (APATT), RSP (Residential Support Program), ICT (Intensive Community Team)) and residential care (Psychogeriatric Nursing Homes (PGNH)). The new Mental Health and Primary Care Partnership Program (MaP) will work alongside our current services to complement and extend supports available to older persons with mental health needs.
As a MaP team member, you will have a clinical role, which incorporates clinical triage (alongside APATT), comprehensive consumer assessment, brief case management, structured psychological support and care co-ordination (alongside primary health clinicians), as well as a role in service development and primary health service capacity building.
The position is 0.5 FTE and operates during office hours.
Classifications for this position will range from PK1 - PK4 ($87, 200 - $98, 200 per annum).
All appointments are made subject to the candidate providing a current satisfactory National Police Certificate conducted by The Victoria Police.
