VincentCare Victoria is seeking an appropriately-qualified person to take on the position of 'families education key worker' within a multidisciplinary team HomeConnect team.
The Families Education Key Worker plays an important role within a multidisciplinary team of professionals in supporting and working with families who are at risk of, or current experiencing housing distress. The role is situated in a team of two other professionals, each with a different focus in supporting and assisting families within the HomeConnect Program. The team works from a "families centred" approach, promoting self-determination and empowerment to achieve sustainable and meaningful outcomes.
The role of the Families Education Key Worker is to work directly with families (including both caregivers and child/children) and schools to maintain engagement, attendance, and to promote healthy social and developmental outcomes for children.
The Families Education Key Worker will act as a key worker to provide practical support, case co-ordination to the family and work collaboratively with the Cross Cohorts team along with external services to achieve the goals identified in the family's Integrated Case Plan.
