Your Role

Join a passionate team where support, collaboration, and career development are key. As a Care Coordinator, you’ll make a real difference by providing high-level, person-centered care to clients with complex needs, including substance use issues. You’ll be a key player in delivering the AOD Continuing and Coordinated Care Program (CCCP), ensuring clients receive specialist care and support for the best outcomes.

Your Responsibility

  • Conduct comprehensive assessments and co-create tailored care plans with clients
  • Collaborate with referral partners and agencies to provide holistic, wrap-around support
  • Establish and maintain referral pathways to connect clients with essential health and community services
  • Ensure culturally appropriate care for Indigenous Australians, linking them with relevant services
  • Maintain accurate client records, case notes, and data
  • Offer direct support in areas such as employment, housing, education, daily living skills, and community reconnection

You Will Need

  • Relevant tertiary qualifications and significant experience in care coordination, particularly in AOD and mental health
  • Proven ability to conduct assessments using standardised tools
  • Experience providing therapeutic interventions and working with clients in a non-judgmental, ethical manner
  • Familiarity with community resources and the ability to work independently and in a team
  • Alignment with the CCCP, the Society’s AOD Model of Care, and Client-Centered Service Delivery
  • Strong computer literacy, especially in data collection
  • A valid NSW driver’s license
