  • SCHCADS Award, Level 5 hourly rates from $40.16 to $42.12 (from Dec 2019) depending on qualifications and experience

About the Service:

The Continuing and Coordinated Care Program is a model of care for people in NSW who are experiencing AOD use issues. The Program's objective is to provide enhanced care coordination and wraparound services for high needs clients who, due to their substance use disorder and co-occurring needs, are facing barriers in accessing substance use treatment or remaining connected to services.

The Role:

The Care Coordinator is responsible for ensuring that those supported by the program receive the care they require and want through direct delivery of support, ensuring care provided by different organisations and services is coordinated and ensuring continuity of care when a person moves from receiving support from one program or service to another.

Key responsibilities of the role will include:

  • Undertaking comprehensive assessment of the clients' complex care needs and developing care plans in consultation with the persons seeking support and with significant others such as family members and friends and other relevant parties
  • Developing, promoting and implementing referral pathways for clients' continuing access to AOD treatment and a wide range of community based /primary health services
  • Facilitating clients' access to external agencies or services that provide assistance with employment, training/education, homelessness/housing
  • Providing direct support and referral to programs that focus on developing daily living skills, promoting / strengthening mental health and general wellbeing; as well as facilitating connection with family and engagement with the community
  • Coordinating additional care and support (direct and/or external) with focus on minimising harms related to alcohol and other drugs, such as Motivational Interviewing and relapse prevention strategies

To be successful in this role, you will need:

  • A relevant tertiary qualification or VET sector qualification
  • Demonstrated experience in coordinating care for persons with complex support needs (including in particular AOD support needs co-occurring with serious mental health issues and other issues such as homelessness, trauma, contact with the criminal justice system and family and cultural separation)
  • Demonstrated ability to undertake assessment including utilising standardised tools
  • Demonstrated ability to work therapeutically with clients (e.g. provide psychosocial support)
  • Demonstrated ability to conduct self in a professional, ethical and non-judgmental manner
  • Demonstrated ability to network and access a range of relevant community resources
  • Demonstrated ability to work as part of a team and to work independently and unsupervised
  • Computer literacy skills including experience in computerised data collection
  • A current, unencumbered Drivers Licence
  • Willingness to work within the values and ethos of the St Vincent de Paul Society and NSW Support Services
