ABOUT THE ROLE

Local Area Coordination Planners (LACs) are committed to improving the lives of people with disability and will work directly with participants to facilitate choice and control over the services they receive.

The LAC Planner will work with the participant (and where appropriate their family and other informal supports) to determine what reasonable and necessary funded supports and services will assist the participant to achieve their goals. They will gather the relevant information, build and develop Participant plans for NDIA approval. The LAC Planner will also work with participants to make changes to their plan through the plan review process which covers scheduled and unscheduled reviews.

The LAC Planner, with assistance from LAC ILC consultants, will support individuals, their families and carers to access mainstream and specialist services in order to meet Participant goals

DUTIES/RESPONSIBILITIES INCLUDE:

  • Work with Participants to gather information required for their Plans.
  • Work closely with participants to identify current and future supports required in order to meet personal goals and achieve desired outcomes.
  • Provide service and community information to Participants to assist them in identify resources or services they require to achieve their goals
  • Providing relevant in-language information and advice on local services and the NDIS.
  • Linking people with Participant readiness activities including SSI LAC capacity building workshops.

SKILLS AND EXPERIENCE

The successful candidate will ideally have lived experience and:

  • Relevant qualifications in the disability sector and/or lived experience or awareness of the impact of existing or acquired disability upon individuals, families, carers and the community
  • Demonstrated experience working in the human services sector e.g. disability, child services, allied health, social work, community development
  • Extensive experience in supporting individuals to establish person-centred support plans in a human services environment
  • Awareness and recognition of cultural diversity, including Aboriginal and Torres Strait Islander and CALD communities
