About the Role

The purpose of this position is to deliver a high quality support service to young people who are transitioning from Care. The role will focus on supporting young people while they are preparing to leave care. The role will also focus on building relationships with relevant service providers which support young people in our community.

  • Ensuring work is planned, managed and reviewed, is consistent with SSI legal and contractual responsibilities, expectations and values.
  • Monitoring the ongoing outcomes for children and young people who have left SSI's care
  • Maintaining contact with young people and their supports in exploring and identifying the underlying issues that have given rise to the need for support
  • Participating in meetings to identify potential skills needed for young people to live successfully independently
  • Ensuring program reports and data to internal and external stakeholders are completed as required and as requested
  • Reporting on progress and outcomes of young people to leadership as required
  • Attending individual supervision and team meetings with relevant programs
  • Participate in individual performance and development plan
  • Undertake consultations with young people on a minimum annual basis
  • Provide advice to caseworkers when consulting with young people leaving care plans
  • Participate in sector forums on Independent living
  • Other duties as directed from time to time within your skills range and capacity.

Selection Criteria

  • Tertiary qualifications (Bachelor Degree) in Social Work, Psychology or other relevant Human Services area (e.g. Teaching, Youth Studies)
  • Demonstrated understanding and commitment to the purpose and values of SSI
  • Expertise in Developmental and sexualised behaviour of children and young people
  • Ability to work from a trauma informed length and strength based approach
  • Ability to manage crisis related behaviour with carers and knowledge of services and support
  • Demonstrated understanding and capacity to high level case management, rapid rapport and engagement strategies and strong understanding of Child Protection and permanency care systems
  • Well-developed skills, including report writing, case notes and the use of a range of electronic data systems and time management skills
  • Demonstrated skills in working with young people who have multiple and complex needs within a range of settings including: Homelessness, A&OD, disability and behavioural challenges
  • Demonstrated understanding of trauma and attachment theories and the impacts on the developing adolescent brain and decision making abilities
  • Well-developed verbal communication skills, experience in effective problem solving, dispute resolution, advocacy for clients, families, carers and service planning
  • Being able to work on weekends when needed
