Healthy Habits Care Coordinator - Part-time to Full-time (up to 37.5 hrs per week). Positions available to be based in our new Community Services Building, Edinburgh North.

Sonder has a 25 year history of delivering primary care services. With a presence across both country and metro SA, our aim is to improve the health and wellbeing of the community by coordinating and integrating health and community services by working in partnership to identify local needs and create strategies in response.

The Healthy Habits Program aims to improve access to local nutrition and physical activity services across regions of Country SA for individuals with chronic disease or health risk factors.

The Healthy Habits Care Coordinator will be required to work collaboratively with clients, health and community services and Healthy Habits Program Facilitators to assess client health needs and facilitate access to appropriate and relevant nutrition and physical activity services.

The role will be responsible for:

  • Working closely with clients, Healthy Habits Program Facilitators, local general practices and other health/community services to support improved access to nutrition and physical activity services, including group programs and individual services
  • Conducting health/risk assessments with clients
  • Developing individual care plans and facilitate access to services to support clients with nutrition and physical activity goals via telehealth
  • Working closely with clients and Healthy Habits Program Facilitators to identify when clients may require additional supports or services and facilitate referrals and access
  • Providing brief motivational interviewing/health coaching interventions to support clients to achieve nutrition and physical activity goals

The Successful Applicant Must Have:

  • Demonstrated experience working within a fast-paced environment
  • Experience in a similar role as a care coordinator
  • Demonstrated experience in working effectively with people with a chronic disease or health risk factors, including development of individual care plans
  • Experience using telehealth platforms including phone and videoconferencing

The successful applicant will be an Allied Health Professional with a clinical understanding of chronic disease. Preference will be given to Registered Nurses.
