Status: Part-time to Full-time (up to 37.5 hrs per week)

Contract Position – funding secured until 30 June 2023

Working Location: Multiple roles are available across the Yorke and Northern and Barossa Hills Fluerieu Local Health Networks. Service hub locations include Nuriootpa, Kapunda, Strathalbyn, Port Pirie, Clare and Kadina. Some outreach to nearby locations will occur from these hubs.

We are seeking passionate and enthusiastic Mental Health Nurses and other appropriately qualified mental health professionals to join our dynamic, multi-disciplinary and growing mental health team to play a crucial role in the delivery of Clinical Care Coordination.

Clinical Care Planners play a key role in helping to provide care coordination for people with severe and complex mental illness managed by GP’s and Psychiatrists in the community. Working within a recovery orientated framework, a joint care plan is developed with the individual and their key personal and professional supports to improve physical health and support effective management of their mental health symptoms and avoid unnecessary hospitalisation. Support is targeted, complementing and enhancing existing GP, psychiatrist and allied mental health services.

Clinical Care and Coordination Program (CCC

The primary goal of this program is to improve access to adequate primary mental health care interventions to maximise recovery and prevent escalation of symptoms through provision of wrap-around coordinated care for people living with severe mental illness and complex needs.

Clinical Care Planners play a key role in helping to provide care coordination for people with severe and complex mental illness managed by GP’s and Psychiatrists in the community. Working within a recovery orientated framework, a joint care plan is developed with the individual and their key personal and professional supports to improve physical health and support effective management of their mental health symptoms and avoid unnecessary hospitalisation. Supported is targeted, complementing and enhancing existing GP, psychiatrist and allied mental health services.

Who can access the program?

  • People diagnosed with a severe and persistent mental health disorder as defined in DSM-5 such as Schizoaffective Disorder, Bi polar Disorder, Personality Disorder
  • Identified as having a clinically severe illness as well as complex multiagency needs, often clinical and non-clinical; or
  • Have been hospitalised at least once for treatment or at risk of needing hospitalisation in the future if appropriate treatment and care is not provided.

Services provided by Clinical Care Planners:

  • Development of individualised multi-agency care plans with the individual (Wellness and Recovery Plan)
  • Provide service coordination with regular follow up and review
  • Support individual to move seamlessly between services as needs change
  • Contribute to addressing physical health needs
  • Support GP’s in their role of assessment
  • Support step up/down and post discharge activities with the Local Hospital Network mental health services.
  • Attendance at multi-disciplinary meetings
  • Support people to effectively manage their symptoms using a recovery orientated approach
  • Enable improved access and coordination across alcohol and other drug and mental health services for clients with co-morbidities.
  • Establish linkages across agencies and services
  • Evaluating the effectiveness of service and treatments provided, including the use of outcome measures
  • Services are provided in offices or in GP surgeries when available

It is beyond the scope of this program to provide acute or inpatient support.
