Location: Sonder Edinburgh North

FTE: 1FTE

The Closing the Gap Integrated Team Care Program Care Coordinator will be responsible for working collaboratively with patients, general practitioners, practice staff and Aboriginal health services to provide appropriate multidisciplinary care and services for Aboriginal and Torres Strait islander people with chronic conditions. All care coordination functions will be in accordance with the care plan developed by a referring GP, with the Care Coordinator driving the implementation of the care coordination model at the local level, utilising existing infrastructure and partnerships to link patients with services needed.

Reporting directly to the Regional Team Leader, your responsibilities will be:

  • Work closely with general practitioners and practice staff to support improved care planning (including compliance) and continuity of care across service boundaries
  • Work closely with the patient and the GP to identify signs when the patient’s condition may need further medical attention and facilitate the patient’s access to a range of care providers and medical aids to support them to remain at home
  • Assist Aboriginal people to understand their chronic health condition and ways to manage it, as well as advising them to follow the recommendations in their care plans
  • Work collaboratively with mainstream and Aboriginal controlled services in the local area, including services provided by state or local government and non-government organisations.

Experience requirements:

  • Demonstrated experience working within a fast-paced environment.
  • Experience in working with Aboriginal communities and the ability to provide culturally appropriate support.
  • Demonstrated experience in working effectively with people with a chronic disease
  • Ability to capture and share clinical information with relevant health care providers, including in electronic format
  • Have a good understanding of the local health system including referral pathways

Qualification requirements:

  • Knowledge and experience in using the Microsoft suite of software and databases.
  • The Care Coordinator will be a health professional with a clinical understanding of chronic disease. The successful applicant should have an AHPRA registration
  • Preference will be given to the following disciplines: Nursing, Aboriginal Health Practitioners and Allied Health
