We are currently seeking suitably experienced Nursing and/or Allied Health professionals to join our highly dedicated team as a Covid Care Navigator.
Silver Chain will be delivering a new Covid Care Navigator Service. You will be working with a supportive and dedicated team in our virtual care centre and will be completing health care assessments, providing support for patients to feel comfortable to manage their care where applicable at home.
Importantly, you will be working to identify signs and symptoms of COVID positive people who may need to go to hospital due to medical deterioration from COVID-19 and as required provide guidance and connection to social and mental health support.
On offer is a 12-month Fixed term contract, Part or Full-time hours.
This is a work from home position, with various shifts available on a 7-day rotating roster between the hours of 8am and 8pm. The flexible nature of this role does not require you to be based in the Eastern states however the shift hours are AEST.
