The opportunity

Kimberley Silver Chain is currently seeking a Case Coordinator to manage local clinical services in the home. Based in the heart of Broome, this position is a pivotal community connection with the local community and health providers, keeping clients living well at home.

This dynamic position is full time and based in Broome. The position involves ensuring the operation of variable funding programs and clinical governance of clinical services provided within the community. Flight to Broome provided at the beginning of contract. Subsided accommodation and utility bills are also on offer.

Key responsibilities

Become part of a small, dedicated and passionate team within Country Health Services reporting directly to the Clinical Manager. You will be accountable for the safe delivery of quality evidenced based clinical practice to the clients in your area, and responsible for the operational competency and performance of clinical staff in your team.

About you

  • Current Registration with the AHPRA
  • Experience in a similar role is desirable
  • Experience in Medical Assessment Wards would be highly regarded
  • Demonstrated experience in leading a team and coordinating resources to achieve outcomes.
  • Demonstrated decision making and problem-solving skills to innovatively manage the provision of quality client care.
  • Demonstrated understanding of contemporary clinical governance knowledge and community nursing practices.
  • Well-developed communication and interpersonal skills, with capacity to negotiate, problem solve and influence individuals and organisations
  • Evidence of a relevant Post Graduate qualification
  • Demonstrated ability to work autonomously with a team focus
  • A current Australian driver’s licence

Under the Aged Care act 1997, successful candidates will be required to provide a satisfactory National Police Clearance prior to commencement.
