Advance Diversity Services (ADS) is a not-for-profit organisation providing support services to people in culturally and Linguistically Diverse (CALD) communities in the South Eastern Sydney region, in particular, St George and Sutherland areas.

We are currently seeking to employ a motivated and passionate person with a commitment to social justice to join our Help@Home team.

The Bilingual Case Manager will support and work with frail aged with their specific needs and goals to maximize their choices and independence. The Bilingual Case Manager is responsible for alignment quality care plans with client-specific needs and effective service delivery through Consumer Directed Care Model (CDC) within a person-centred, wellness & enablement approach and client satisfaction.  

All applicants must obtain a job information package and submit a written application addressing each of the following selection criteria.

Essential Criteria:

  1. Tertiary qualifications in social work, aged care, allied health or demonstrated competence/ experience.
  2. Demonstrated experience and have worked in assessment, case management and reporting roles within the community aged care sector.
  3. Proven knowledge of consumer-directed care and understanding of community aged care services and its requirements.
  4. Strong communicator, both written and verbal. Be able to speak fluently in English and Italian.
  5. Sound computer literacy including CRM (Client Relationship Managers software).
  6. Current NSW driver’s license, and have access to a car for work with the ability to travel regularly.
  7. Current National Criminal History Record Check and show evidence of the eligibility to work in Australia.
  8. Physically fit to fulfil the physical requirements of the role.

Desirable Criteria:

  1. Demonstrated skill  in CALD community engagement
  2. Current First Aid certificate
