The Program

The new Commonwealth Psychosocial program, Connector Hub, is commissioned by South West Sydney Primary Health Network to provide time-limited support to individuals living with severe and persistent mental health challenges to:

  • connect with services
  • receive 1:1 recovery support
  • have access to skill building and recovery focused group support, and
  • receive support to test their eligibility for NDIS.

About the Role

This role works with people with psychosocial disability to facilitate:

  • Culturally informed intake and referral, in a manner which focuses on wellbeing and recovery.
  • Timely access to the best options for support at One Door, eg NPS Access, Connector and NDIS Services
  • Timely referral to external programs for support, if One Door is not the best option.

This role works with teams to assist the program workflow in providing supports for and with consumers. One Door operates locally as self-organised teams – called Recovery 2gether teams. Team members are collectively responsible and accountable for supporting consumers and team tasks.

Key responsibilities include:

  1. Triage incoming referrals for Connector Hub South West Sydney, and or other service options based on consumers support needs.
  2. Perform brief, recovery-focused intake assessments to match consumer-need with program eligibility
  3. Warmly connect consumers with the best support option (internal and external) to match need.
  4. Record timely consumer data, case notes, outcomes in the Client Management System.
  5. Form and maintain relationships with key psychosocial support stakeholders; attend relevant meetings.
  6. Maintain and act upon up-to-date information on available NDIS programs for each region we service.

To succeed in this role, we are looking for:

  • An ability to exercise personal responsibility and work as part of a self-managing team, with the guidance of the Coach and/or Transition Manager
  • Values consistent with person-centred ways of working and supporting the person’s choice and control
  • Relevant tertiary qualifications in mental health, health or welfare degree, or Cert IV in mental health or equivalent level of expertise attained through previous appointments, service and study
  • Knowledge of NDIS
  • Experience in mental health
  • Ability to use culturally safe practices in supporting Aboriginal and Torres Strait Islander peoples
  • Ability to work with culturally and linguistically diverse peoples
  • Ability to work with people who identify as LGBTQIA.
  • Strong communication
