Partner with primary health and community health providers to address both the physical and social determinants of health for at-risk members of the community.

Reporting to an independent Board, SEMPHN is funded by the Australian Government to help people in south east Melbourne get the health care they need, when and where they need it.

The successful candidate will be flexible and a confident self-starter, with excellent project management skills, an ability to engage with and build strong relationships with diverse stakeholders, and with relevant experience in the Chronic Disease sector.
You will join a cohesive, supportive Service Development Chronic Disease team. This role will effectively manage provider contracts, as well as working collaboratively with our stakeholders and consumers to support the commissioning of solutions in chronic disease through phased strategic project planning, identification of gaps in health services, the co-design of new models of care as well as ensuring relevant outcome frameworks are determined that will measure performance.
